In 2013, we will continue to integrate the Health Promotion and Wellness rewards into the Employee Rewards and Recognition Program.
Employees will receive points that can be redeemed for a variety of gift options, or accumulated over time toward the purchase of more expensive gifts.
Each employee is eligible for up to $200 worth of points/rewards over the course of the calendar year.
Know Your Health Coach
|Low Risk participants
||Meet with health coach once per year. No need to see health coach for follow-up unless participant wants to. Check-in at 6 months is optional.
||All points given at annual encounter; $200/person.
|Medium Risk participants
||Complete at least two face-to-face follow-up encounters with health coach during the year, each approximately 6 months apart. /td>
||$100 per encounter/person. /td>
|High Risk participants
||Meet with health coach 3 times per year every 4 months.
||$50/person for 1st & 3rd encounters; $75 for 2nd.

The total amount an eligible employee can earn in a year is $200 worth of points. Per diem employees can earn a lesser amount.
Low Risk: If your health risk assessment results place you in a low-risk category, you do not need to meet with a health coach again during the year. Instead, you should submit your progress reports by fax (or interoffice mail) to Health Promotion and Wellness (15 Enterprise Drive, Augusta) for review. If you want to be supported by a health coach, you can still sign up for an additional appointment.
Medium Risk: If your health risk assessment results place you in a medium-risk category, you should meet with a health coach again approximately 6 months. Your personal goals will be reviewed in person and you will not need to meet with a health coach again for the rest of the year.
High Risk: If your health risk assessment results place you in a high-risk category, you will meet with a health coach approximately 4 months following your annual HRA and again 4 months after that visit, for a total of 3 encounters per year. Your personal goals will be reviewed in person during each meeting.
Key Points to Remember
- In 2013, points will only be rewarded when you meet with one of our health coaches.
- Points will not be awarded for taking your health risk assessment online, getting preventive dental care or demonstrating that you have seen your primary care provider this year.
- There will be opportunities to earn additional points/rewards during quarterly wellness programs we will be offering.
