Living Well Chronic Disease Self-Management

Living Well is a workshop series that helps people better cope and manage their long term health issues.

These fun workshops are two hours a week for six weeks and are held in doctors' offices and other places in your area.

Anyone living with or caring for someone with a long term health issue is welcome to attend.

Workshops are led by two trained leaders; one or both of whom are dealing with a health issue.

The workshop covers ways to deal with problems such as:
  • Being upset
  • Being tired
  • Feeling sad
  • Having pain
  • Being inactive
  • Being stiff
  • Using medicines
  • Talking with others
  • Eating well


Setting goals, working with others to get ideas and solve concerns is done each week


Program Goals

  • To reduce problems from long term health issues
  • To increase healthy ways of taking care of oneself
  • To reduce health care costs
