Community Support and Assertive Community Treatment (ACT)


For people with persistent mental illness, the ACT Team provides access to a range of intensive services and resources.

We help people access the wellness and recovery resources they need.

The ACT program is a specialized team that includes a psychiatrist, psychiatric mental health nurse practitioner, psychiatric nurses, a vocational specialist, a substance abuse specialist and community support workers that provide consistent and frequent treatment to people with persistent mental illness who need more intensive support to remain in the community and avoid hospitalization.

The team connects clients with needed services and provides psychiatric medication and monitoring, treatment groups and case management services.

Our services and support allow those with persistent mental illness to be successful members of the community.
