We're located in a quiet, wooded area only minutes away from the conveniences of Augusta. Gray Birch is easy to get to and provides a beautiful, scenic living environment.

On July 13, we restarted outdoor, in-person visits in a limited, safe way for all. Starting Oct. 26, we will begin indoor visits, as well. Learn more.
Events at Gray Birch
The Gray Birch Activities staff are committed to keeping residents engaged and entertained through the pandemic.
During the warmer days they are offering outdoor entertainment.
Church and religious services are continuing via zoom with residents masking and/or social distancing.
The team will continue to offer outdoor visits with families as weather allows. Call 621-7130 to schedule your visit.
Family members and visitors are encouraged to bring in outdoor clothing as the weather gets cooler. To schedule an outdoor visit call the activity department.
Our facilities will be transitioning to indoor visits as soon as we can! Due to COVID-19 restrictions the process for scheduling visits will remain the same through the activity department.
Skilled Nursing Care and Rehabilitation
Our experienced, caring team of nurses and therapists are committed to offering patients the excellent care they need to return home as soon as possible.
Our staff is dedicated to making every patient's stay comfortable and successful while helping them regain independence in a supportive, welcoming environment.
Assisted Living - The Residence
For seniors who can function relatively independently but require a supportive environment, Gray Birch offers The Residence at Gray Birch, a residential care community.
Our focus is to be resident- and family-centered in our services. We engage with residents and strive to create a pleasant, home-like casual living environment.
We treat residents as the unique people they are, with varying levels of care. We'll work with you on customized service plans to meet your physical, emotional and social needs.
- 24-hour staffing support
- Life-enhancing activities, seven days a week, with many community outings
- Dining services that are pleasant with nutritious meals
- Laundry and housekeeping services
- Physical, occupational and speech therapy as needed
- Respite care for family caregivers
Gray Birch has a 37-bed capacity, offering three private rooms and 17 semi-private rooms.
Our day room allows families to gather and residents to participate in activities or enjoy quiet time. We also have an alcove where residents gather and socialize.
The Residence at Gray Birch offers wireless Internet access for residents and their family members.
Availability of Skilled Care
If your loved one needs skilled nursing services after a hospital stay, we can transfer them from the hospital to the Skilled Unit on the floor below our "neighborhood." This option allows skilled staff to meet your loved one's nursing care needs while we maintain a bond with them.
"No Boundaries" Activity Program
Our activity and wellness department prides itself on its "No Boundaries" program. Each resident has a personal activity program.
If a resident wants to participate in an activity, we'll find a way to make it happen. Social and physical programs also are available to meet our residents' individual and group goals.
Our large dining room offers restaurant-style dining. Residents have many choices at meal times. They can decide what time they wish to eat and can choose a meal plan to meets their nutritional needs.
All of our meals are prepared with homemade ingredients and care to creating a meals similar to what our residents would prepare for themselves.
Long-term Nursing Care
We also offer long-term nursing care at Gray Birch. Individuals who need a longer stay and need extensive help with activities of daily life can choose to stay in our long-term care community. Residents can take advantage of a wide variety of choices and activities daily.
We readily accept volunteers at Gray Birch and welcome what they add to residents' lives and our program. Volunteers can help in many ways, including:
- Helping with activities or outings
- Providing companionship to residents
- Leading small discussion or activity groups
- Coordinating projects
- Providing entertainment
