KRHA operates as a physician hospital organization which consists of a collaborative network that includes independent practices as well as MaineGeneral-affiliated practices. Kennebec Valley Community Care Team
Today’s health care system can be confusing and hard to manage.
The Kennebec Valley Community Care Team (KVCCT) is here to help you and your family understand your care and get the support you need. We can help if you or someone you care for has:
- Many health problems;
- Trouble managing the things your primary care practitioner (PCP) says may help you feel better; or
- Family, school or social-related needs.
We work with children, adults and seniors to make sure everyone gets the best care possible.
What We Do
We work with your PCP and specialists to help you:
- Feel better and stay healthy
- Lower medical costs
- Reach your health goals
Our Staff Will:

Learn about your health needs and goals;

Work with you and your family to manage your care and medicine;

Teach you about your health when you need help or have questions;

Help you set goals and stay on track;

Connect you with the right services for school, home or community;

Work with other doctors and agencies to make sure everyone is on the same page;

Support you at medical visits if you'd like; and

Help you understand health information.
