The state of Maine requires every individual entering a nursing facility to be evaluated by a registered nurse assessor to determine the level of care. Our social workers will assist you with that process.
Once you have decided to apply for admission, a social worker will discuss financial concerns with you.
Following admittance, the interdisciplinary team (IDT) develops a plan of care along with the resident's primary physician.
The IDT consists of the resident, family members, a social worker, a dietitian, nursing staff and the facility's activity coordinator.
Discharge planning begins upon admission. For all residents, we focus on restorative measures.
When discharge is appropriate, we work that goal into the plan of care.
