The HealthReach Telehomecare Program uses home-based monitoring devices and telephone connections to link home health nurses directly with patients in their homes.
This does not replace the nurse but adds to the care and monitoring patients receive.
Your care will be the same whether the nurse comes to your home or "visits" via this system.
You will have a combination of in-person and televisits to make sure you are getting the care you want and need.
The program:
- Is not an emergency service
- Monitors patient vital signs daily
- Maintains patient privacy at all times
- Automatically updates HomeCare medical records electronically
- Provides health data to the patient's physician
- Is easy to use and help decrease patient hospitalization
- Is available at no additional cost to eligible patients
Program Eligibility
There are several eligibility requirements for the Telehomecare Program.
- Call us to inquire whether telehomecare is an appropriate complement to your regular in-person visits.
- You must be HomeCare-eligible to receive this service.
- Your doctor must be involved in this care and informed of changes in your health.
- If you qualify, a nurse will bring the equipment and show you how to use it.
