Through our program, we provide you with a convenient way of meeting with several of your providers in one place, saving you time, energy and expense.
The setting also allows providers to better coordinate your care by sharing each other's professional knowledge in a timely way, thus avoiding lost time in meeting your healthcare needs.
By having access to your primary care doctor, mental health specialists, case managers, nutritionists and others in one location, you can get many of your needs addressed and questions answered all at once.
We follow an interdisciplinary team approach to providing care and you are the coach of this team.
We will meet to discuss your current needs and develop a coordinated plan of care.
If, in meeting with your providers, there are things you would kept just between you and that provider, please let us know.
We hope this arrangement provides a comprehensive approach to meeting your needs, particularly if you experience complications with your condition.
