Horizon Program

The Horizon Program's mission is to provide quality, compassionate and comprehensive medical and social services to all identified HIV-positive individuals in central Maine.

Based out of MaineGeneral's Gardiner Campus, the Horizon Program provides:

  • A clinic for medical services and interaction with healthcare professionals

  • Case Management for people living with HIV/AIDS

  • Educational programs and community outreach

  • Counseling and support for people infected and affected by HIV/AIDS

Program History
MaineGeneral Health Associates (MGHA) was funded by the Ryan White Title III EIS Program in 2003 to enhance existing systems of care and decrease fragmentation of HIV services.

Funding also was intended to increase access to and quality of care to improve health outcomes for individuals living with HIV/AIDS.

In September 2005, MGHA received a Title III Capacity Building Grant to:

  • Overhaul its existing electronic medical record (EMR) system for quality improvement and assurance

  • Establish a Peer Advocate and Peer Educator Program to decrease client social isolation and foster a peer-provider alliance

  • Provide startup funding for a monthly HIV clinic

In early 2007, the Ryan White Title II and Title III programs were renamed Part B and Part C, respectively, and the Horizon Program assumed responsibility for administering both parts.

Having one program coordinating both parts is expected to decrease fragmentation of services and lead to improved quality of care and increased effectiveness and efficiency in healthcare delivery.

Community Involvement
Community support is vital to our program's ability to provide needed services. We need committed volunteers to help us in the office or out in the community.

We welcome serious inquiries from people interested in becoming involved.

Volunteer opportunties include office support, fund raising, peer education, client buddying and condom distribution. 

Funding Information 
Our program is supported by grants from federal sources, the Maine Department of Health and Human Services, the United Way and donations from individuals.
